This article aims to understand the intersubjective, group, institutional, and political dynamics that permeate the hospital context and their implications for the care bond, from a psychosocial and psychoanalytic perspective. The results are discussed across three domains: the political and transinstitutional, the institutional, and the intersubjective. Regarding the first domain, political and transinstitutional aspects, we explore the transitions of power structures in the hospital, articulating them with the nature of institutions to understand how historical and structural transformations in the hospital field have significantly impacted relational configurations of that environment, hindering humanization and tolerance in situations of extreme suffering. Secondly, regarding the institutional sphere, we discuss the institutional foundations of the hospital through the lens of psychosocial and psychoanalytic theorization. Finally, we reflect on how, at the intersubjective level, the inability to deal with experiences of helplessness—often present among healthcare professionals—compromises the essential psychic bond needed to build care relationships between caregivers and patients. These challenges highlight the importance of psychologists considering different institutional dimensions as fundamental to understanding the difficulties encountered in care relationships and proposing strategies that promote the humanization of experiences in this context.
